This is a great idea and allows the student to save face. I'm just wondering what the children do with them when it's at the table. What's your system?
I used these warning cards with my young elementary students when they needed a reminder them to make better choices in art. It was very useful for kindergarten. You could fill the shapes with red in Word before printing but I just printed them as is on red card stock, laminated, and then cut them out.

I have the student keep it at the table as a visual reminder for the remainder of the class. At the end of class the student hands it back to me and we have a chat about why they got the warning and what we need to do differently next class. If a student was still having problems after receiving the card I will walk by and tap the card as a 2nd warning but anything beyond that would require leaving their table to take a break.
