I use this worksheet with students who are common trouble-makers in art class. Older students fill it out on their own, younger students fill it out with me. Worksheet goes home to be signed by parents and must be returned for documentation.

    I'm a little surprised that you can use the "being a bully" line. In my district bullying is taken so seriously and must be a heavily documented thing. You can not tell students that they are behaving like bullies because their parents can sue for defamation if the kid goes home and says the teacher said you called them a bully.
