Art Room Helper Application

I have used 5th Grade helpers at the end of the school day to help manage Art Room organization and general clean up. Here is my 5th grade helper application. If it is helpful to you, feel free to use/alter it! It has helped my anxiety and made it easier to handle the every day chaos in the Art Room.

    Just what I was looking for! Do you have a difficult time having the 5th grade classroom teachers release their students consistently to come do their helper job?
