Year-Long Behavior Self-Assessment Chart

This form is what I use for my 1st grade through 5th grade students to chart their behavior in Art. At the beginning of each quarter they make a goal. At the end of each week's class, the students color in how they think they did in art that day. The class rules are at the bottom. This is an Excel file that can be customized with your own rules and weeks you see students. It's a great self-assessment tool.

    This is a great idea for self-reflection. I may use this next year to help my students stay on track for success in my art room.
